Why do I use the wrong form of words?

When I am talking I will use the wrong form of a word. I will know it is wrong as soon as it is coming out of my mouth, and I will correct myself. Ex: I got a bad sunburn, I am going to PALE so bad. When I meant PEEL. I mix all of them up. Just when I am talking, I write them down correctly. Beer/Bear Pair/Pear Hair/Here...etc. I know it is the wrong one in my head, but it always comes out wrong anyway!!! What is wrong with me? This isn't sudden, I've been doing this for as long as I can remember. It is really beginning to bother me now, because I am 27, I have an 8 month old baby that I don't want to confuse with my crazy talk!! I am thinking some sort of dyslexia??
